Sounds to me like he didn't tighten it correctly, if at all. or perhaps he over torqued it and damaged the mount. I'd take it back to him and ask him to recheck the engine mount. if he over torqued it then HE should have to replace it. if it's not tight enough then he should just tighten it back up
 
Over torqueing will ruin ANY car part, not just mounts. that is why basically every single bolt has a factory specified torque and why the use of a torque wrench is heavilly suggested when working on cars. so basically from the sounds of things, your mechanic either used a normal impact wrench or a impact torque wrench and didn't know the correct torque specifications. he clearly over torqued it, as time and vibrations would not cause this indentation - time and vibrations would cause the mount to loosen and to fail but definitely not said indentation
